About the Role

LHC Group is seeking a skilled Physical Therapist to join our Home Health team in Portland, Oregon. As a Physical Therapist, you will be responsible for assessing and evaluating patient care needs, developing treatment plans, and providing interventions to improve patient well-being.

Responsibilities
  • Evaluate patient functional status and physical therapy needs
  • Assist the physician in developing the therapy plan of care
  • Observe and report patient response to treatment and any changes in condition to the supervising nurse and/or physician
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice
  • Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals
Requirements
  • Current Physical Therapy licensure in the state of Oregon
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
