Job Summary

We are seeking a qualified and caring ESN QIDP/Case Manager to join our team in Fort Wayne. As a Case Manager, you will provide case management services to residents in our ESN group homes, including administering assessments, overseeing resident program plans, and coordinating delivery of services.

Responsibilities
  • Provide case management services to residents in ESN group homes, including assessments, program planning, and service coordination.
  • Develop and monitor individual plans for clients, ensuring compliance with state and federal regulations.
  • Monitor and maintain client and group home files, ensuring all documentation is current.
  • Participate in recreational, therapeutic, and training activities with clients.
  • Communicate effectively with IDT and supervisor, seeking input and feedback as needed.
  • Safeguard client rights and coordinate with the Human Rights Committee as required.
  • Provide training and supervision to staff, ensuring compliance with state and federal regulations and Benchmark Human Services policies.
  • Monitor client health care needs, including plans, medications, and treatment protocols.
  • Attend agency and departmental meetings, client appointments, and chair client-specific and house meetings.
  • Provide on-call services and respond to client and house needs.
Requirements
  • Reliable transportation and valid driver's license.
  • 4-year degree in a Human Services field.
  • Minimum 1 year experience working with DD clients.
