About the Role

Bellefaire JCB is a leading child service agency providing a range of mental health, substance abuse, education, and prevention services. As a Care Coordination Specialist, you will play a crucial role in engaging clients and their families to offer care coordination services through OhioRISE and initiate the completion of the CANS assessment and intake paperwork.

Key Responsibilities
  • Initiate Clinical Assessment: Conduct verbal, written, and/or face-to-face interviews to determine the appropriate level of treatment for clients.
  • Provide Face-to-Face Admission: Complete all required clinical and administrative paperwork for clients and guardians.
  • Make Recommendations: Suggest agencies in the community or across the country if Bellefaire JCB is unable to cover the needed service.
  • Assist in Departmental Coverage: Manage a diverse caseload of all types of referrals, representing the Agency's service array.
  • Other Duties: Perform other duties as assigned by management.
Qualifications
  • A Bachelor's Degree is required. A Master's Degree in Social Work or a related field is preferred.
  • Valid Ohio LSW, LISW, or PCC is preferred.
  • At least 3 years of experience in the mental health field is required. Experience working with youth and their families is preferred.
