ACT Case Manager Opportunity

Spurwink is seeking a highly motivated and compassionate ACT Case Manager to join our interdisciplinary team serving adults with serious mental illness in the greater Portland area.

About the Role

This is a challenging and rewarding position that requires a strong understanding of mental health principles and a commitment to providing high-quality care. As an ACT Case Manager, you will work closely with a team of professionals to develop and implement individualized treatment plans that address the unique needs of each client.

Key Responsibilities
  • Conduct comprehensive assessments and develop individualized treatment plans in collaboration with the treatment team.
  • Provide ongoing support and guidance to clients and their families, including regular phone contact and face-to-face meetings.
  • Collaborate with community providers and other stakeholders to ensure seamless care coordination.
  • Participate in team-based approach to care, including regular team meetings and case consultations.
  • Identify and assess physical and behavioral health-related risk factors and ensure that clients and families are aware of all community services and resources.
Requirements
  • MHRT-C certification or MHRT-C eligible, or a Certified Rehabilitation Counselor or Employment Specialist, or a Certified or Licensed Alcohol and Drug Counselor.
  • Bachelor's degree from an accredited four-year institution of higher learning preferred.

  • A competitive hourly rate of $21.00 to $27.00, depending on experience.
