Job Title: Medical Social Worker - Home Health Per Diem

About the Role:

The Medical Social Worker plays a vital role in the interdisciplinary care team, providing emotional and social support to patients and their families in the comfort of their own homes. As a Medical Social Worker, you will assess patients' social and emotional needs, develop personalized plans of care, and collaborate with healthcare professionals to ensure comprehensive care.

Key Responsibilities:

  • Conduct thorough assessments of patients' social and emotional needs, identifying areas for improvement and developing targeted interventions.
  • Collaborate with healthcare professionals to develop and implement individualized plans of care, ensuring patients receive comprehensive support.
  • Provide emotional support and counseling to patients and their families, addressing concerns and promoting coping strategies.
  • Identify and connect patients with community resources, facilitating access to essential services and support.
  • Document patient progress, maintaining accurate and detailed records of care provided.
  • Participate in interdisciplinary care team meetings, contributing to the development of patient care plans and ensuring seamless communication among team members.

Requirements:

  • Masters or doctoral degree in social work from an accredited institution.
  • Current licensure as a social worker in the state of practice.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Proof of current CPR certification.
  • Minimum one year of experience as a social worker in a healthcare setting, home health, and/or hospice.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, prioritizing tasks and managing multiple responsibilities.
  • Flexibility to work varied schedules, including evenings and weekends, as needed.
