Requirements and Qualifications

To be successful in this role, candidates must possess:

  • A Master's or doctoral degree in Social Work from a Council on Social Work Education-accredited program.
  • Current licensure as a Social Worker in the state of practice, as required by law or regulation.
  • A valid driver's license, auto insurance, and reliable transportation.
  • Proof of current CPR certification.
  • A minimum of one year of experience as a Social Worker in a healthcare setting, home health, or hospice.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Knowledge of community resources, discharge planning, and social work principles.
