Job Title: Hospice Social Worker

We are seeking a compassionate and skilled Hospice Social Worker to join our team at Interim HealthCare Home Health and Hospice. As a Hospice Social Worker, you will play a vital role in providing emotional, social, and spiritual support to patients and their families.

Key Responsibilities:
  • Conduct thorough psychosocial assessments of patients and families to identify their needs and develop a comprehensive plan of care.
  • Provide individual and family counseling to address mental and emotional health concerns.
  • Identify and address special needs related to cultural and religious diversity.
  • Connect patients and families with community resources and provide follow-up support.
  • Participate in the development of the Interdisciplinary Group (IDG) plan of care and attend regular meetings.
Requirements:
  • Master's degree in Social Work from a Council on Social Work Education-accredited program.
  • Active licensure as a Social Worker in the state of employment.
  • Minimum one year of experience in healthcare, with hospice or homecare experience preferred.
  • Reliable transportation and a valid driver's license.
