Job Title: LVN Charge Nurse

Regency Integrated Health Services is seeking a skilled LVN Charge Nurse to join our team at Windsor Mission Oaks - San Antonio, TX.

Job Summary:

The LVN Charge Nurse will be responsible for ensuring the provision of quality care that promotes the highest practicable physical, mental, and psychological well-being of each resident. This includes observing, assessing, documenting, and reporting resident condition/changes, as well as implementing care plans as directed.

Key Responsibilities:
  • Conduct regular rounds to observe and assess resident condition/changes
  • Implement care plans as directed by physicians and nursing staff
  • Administer medications as prescribed by physicians
  • Conduct rounds with physicians and record visits in clinical records
  • Perform narcotic reconciliation with incoming and outgoing shifts
  • Identify significant changes in resident condition and take necessary action per facility policies and procedures
  • Oversee and monitor aides to ensure quality care is provided
  • Initiate and complete Incident and Accident Reports per facility policy
  • Discharge residents per facility policy
  • Communicate with resident's point of contact regarding status updates or changes in condition
Requirements:
  • Active Licensed Vocational Nurse in the state of Texas in good standing
  • Experience working in a skilled nursing facility preferred
