Job SummaryThe LVN Charge Nurse will be responsible for ensuring the provision of quality care that promotes the highest practicable physical, mental, and psychological well-being of each resident. This includes observing, assessing, documenting, and reporting resident condition/changes, administering medications as prescribed, and conducting rounds with physicians.
Key Responsibilities- Observe and assess resident condition/changes during rounds
- Implement care plans as directed by physicians and nursing staff
- Administer medications as prescribed by physicians
- Conduct rounds with physicians and record visits in clinical records
- Perform narcotic reconciliation with incoming and outgoing shifts
- Identify significant changes in resident condition and take necessary action per facility policies and procedures
- Oversee and monitor aides
- Initiate and complete Incident and Accident Reports per facility policy
- Discharge residents per facility policy
- Communicate with resident's point of contact when they ask for a status update or if there is a change in condition
- Active Licensed Vocational Nurse in the state of Texas in good standing
- Experience working in a skilled nursing facility is preferred
