Key Responsibilities:

  • Coordinate home care services for participants using home care agencies
  • Participate as a member of the Interdisciplinary Team (IDT)
  • Conduct nursing care within LVN scope of practice
  • Provide general nursing care in the clinic and participant's home
  • Document in the EMR within set documentation timeline
  • Communicate with home care employees to understand participant's condition
  • Collaborate with the Transportation Coordinator for participant transportation
  • Schedule home care appointments and support scheduler for other appointment needs
  • Oversee visits completed by external agencies

Requirements:

  • 3+ years of experience in geriatric care and home health care
  • 4+ years of LVN experience
  • LVN License obtained in CA
  • BLS from American Heart Association/American Red Cross
  • CA Drivers License
  • Good driving record & Auto insurance
