About the Role

We are seeking a compassionate and skilled Certified Home Health Aide to join our team at Modivcare. As a Home Care Professional, you will play a vital role in providing high-quality care and support to our clients in their homes and communities.

Key Responsibilities
  • Provide professional, non-clinical personal care and caregiving services to enable clients to remain healthy and independent in their home and community settings.
  • Assist with instrumental activities of daily living, such as bathing, grooming, dressing, and toileting.
  • Encourage self-care and independence in clients, promoting their overall well-being and quality of life.
  • Maintain a clean, comfortable, and safe living environment for clients.
  • Transport clients, as requested or approved, to appointments and other destinations.
  • Prepare meals and perform household tasks, such as housekeeping and laundry.
  • Support clients in completing activities of daily living, including grocery shopping and managing medication.
  • Contribute to a client's quality of life by influencing patient safety, good hygiene, healthy eating, medication adherence, and clean home environments.
  • Decrease social isolation through companionship and engage clients in daily activities to improve mental health.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
Requirements
  • Desire to contribute to the well-being and quality of life of our clients.
  • Competent in providing direct care to clients, including bathing, grooming, dressing, toileting, and personal hygiene improvement and maintenance.
  • Ability to work independently and as part of a team to provide comprehensive care services.
  • Excellent communication and interpersonal skills, with the ability to navigate diverse client personalities and behaviors.
  • Ability to identify and escalate unsafe or inappropriate home and community conditions.
  • Ability to escalate life-threatening emergency situations calmly and appropriately.
