Job Summary

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team at I Am Boundless, Inc. as a Health Services Manager. In this role, you will be responsible for overseeing the management of health care for individuals living on our grounds, ensuring that their medical, wellness needs, and personal outcomes are met.

Key Responsibilities
  • Provide overall management of health services, including identification of health and wellness needs and development of plans to meet these needs.
  • Serve as the medical liaison with individuals served, families, physicians, and significant others.
  • Provide training and observations related to Medication Administration, administering G-tube nutrition and insulin, and other health care factors and treatment.
  • Review medical and nursing practices and required documentation requirements, ensuring that the information in the medical record is maintained and meets regulatory standards.
  • Monitor and approve compliance of all DODD, ODH, and other regulatory requirements, and participate in completing Plans of Correction.
  • Provide medical and wellness input to individuals served, team members, and families/guardians, and assist the team with goals associated with health and wellness.
  • Participate in the admission of new residents, coordinate medical and wellness services with healthcare providers outside of Boundless, and provide medical and wellness input to various assigned committees for strategic planning, policy development, and quality assurance.
  • Attend meetings and training as necessary, complete quarterly health status reviews on assigned residents, and attend medical appointments as needed.
Requirements
  • Must be at least 18 years of age.
  • Must be able to communicate in English, including both spoken and in writing.
  • Current and in good standing LPN License with the Ohio State Board of Nursing.
  • First Aid, CPR - adult, child, and infant; or the ability to become certified in CPR/FA within 60 days of hire.
  • Valid Ohio Driver's License with Ohio Bureau of Motor Vehicles; proof of auto insurance; insurable driving record (five points or fewer).
Preferred Qualifications
  • One year's experience working as an LPN with individuals with behavioral health conditions or intellectual/developmental disabilities.
Work Environment

This job operates in a professional office environment, and the employee will be exposed to virus, disease, and infection from patients and specimens in working that environment or other clinical environments.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand, walk, and reach with hands and arms.

This position requires intermittent physical activity, including standing, walking, bending, kneeling, stooping, and crouching as well as lifting and supporting patients.

The essential sensory and communicative activities include: Feeling, Seeing, Speaking, Hearing.

An individual in this position may be exposed to the following environmental conditions when performing duties: Inside Environmental Conditions, Outside Environmental Conditions, Exposure to Blood, Exposure to Bodily Fluids, Combative individuals.

Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected on occasion.
