Job Summary
As a Home Health Physical Therapist, you will be responsible for planning and conducting segments of treatment programs designed to restore and improve physical, social, and mental functions while meeting department objectives. You will also serve as a co-leader in an interdisciplinary approach to patient groups. Your expertise will play a crucial role in ensuring that patients receive high-quality care in the comfort of their own homes.

Responsibilities
This role requires a Bachelor's Degree or equivalent and physical therapy licensure in the state of practice. Certification in CPR or Basic Life Support is also necessary. As a key member of our team, your responsibilities will include:
