**Job Summary:**

We are looking for a dedicated and compassionate Physical Therapist to provide high-quality patient care as a Home Health Physical Therapist. As a member of our team, you will be responsible for evaluating patients, developing personalized treatment plans, and delivering evidence-based therapies to promote optimal health outcomes.

The ideal candidate will have excellent communication skills, a strong understanding of home health physical therapy principles, and the ability to work independently with minimal supervision.

**Key Responsibilities:**

* Evaluate patients in their homes to assess their physical therapy needs
* Develop and implement individualized treatment plans to address patients' goals and objectives
* Provide direct patient care, including exercises, stretches, and other interventions as prescribed
* Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination

**Requirements:**

* Current Maine Physical Therapy License
* Recent experience working as a Home Health Physical Therapist (minimum 3 years)
* Valid BLS/CPR certification
