Job Summary

Home Services Unlimited is a leading independent Medicare-certified agency seeking an experienced home health nurse to serve Indianapolis. As a key member of our team, you will directly manage patients in the field, advocate for their welfare, and serve as a liaison between patients, their families, and healthcare providers.

Responsibilities
  • Perform assessments and provide direct nursing care to patients in their homes.
  • Develop, implement, and evaluate individualized patient care plans.
  • Coordinate patient care in a timely manner and promote personal safety and a safe environment for patients and coworkers.
  • Implement appropriate standard precautions and other infection control practices.
  • Supervise LPN and/or paraprofessional staff per policy.
  • Complete and submit required documentation in an accurate, complete, and timely manner.
  • Promote effective communication, both verbal and written.
  • Maintain expected standards of quality care within the scope of professional nursing practice.
  • Participate in meetings, in-services, and activities as required by the agency.
  • Seek assistance appropriately and comply with agency policies and procedures.
Requirements
  • Prior home health experience, including OASIS.
  • Current Indiana RN license.
  • Associate degree or a bachelor's degree from an accredited nursing program.
  • Previous experience with electronic health records is preferred.
