Job Description:

As a Prevention Navigator at CHASI's Prevention Department, you will play a vital role in addressing health disparities by coordinating outreach, education, and referrals for priority populations. You will provide case management and referrals services for clients in target populations to offer comprehensive HIV prevention services. This position requires flexibility to work during evening hours and on weekends.

Key Responsibilities:
  • Conduct intakes, mental health screenings, substance use assessments, and other evaluations to identify client needs
  • Develop and integrate service plans with clients, providing education and counseling on prevention strategies and materials
  • Assist clients with appointment scheduling, provider follow-ups, and transportation as needed
  • Perform effective case management from intake to discharge, tracking client progress and maintaining accurate records
  • Provide linkage and coordination with internal and external services, including PrEP, emergency PEP, iART/HIV care, STI treatment, and ensure successful referrals


Requirements:

To be successful in this role, you must have at least 2 years of experience providing case management or health education to high-risk individuals, preferably with immigrant or LGBTQ+ populations. You should possess a high school diploma or equivalent, with some college preferred. Bilingual skills in English/Spanish are required, along with basic knowledge of HIV, HCV, STI, PEP/PrEP, and Harm Reduction methods. Excellent MS Office skills and ability to work independently and collaboratively with diverse teams are essential.Strong verbal and written communication skills, including listening and following instructions, giving direction, and following up on tasks
