Job Summary

We are seeking a skilled and compassionate Physical Therapist to join our team at Self-Help For The Elderly. As a key member of our healthcare staff, you will be responsible for providing high-quality physical therapy services to patients in their homes.

About the Role

The successful candidate will have a strong foundation in physical therapy principles and practices, with a focus on delivering patient-centered care. You will work closely with our interdisciplinary team to develop and implement individualized plans of care that address each patient's unique needs and goals.

Key Responsibilities
  • Conduct thorough assessments of patients' physical functioning and mobility to identify areas of need and develop targeted treatment plans
  • Provide one-on-one physical therapy sessions to patients in their homes, using evidence-based techniques and interventions to promote recovery and independence
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care and optimal patient outcomes
  • Document patient progress and adjust treatment plans as needed to ensure continuous improvement
  • Maintain accurate and up-to-date records of patient interactions, including assessment results, treatment plans, and progress notes
Requirements

To be considered for this role, you must possess:

  • A degree from an accredited physical therapy program
  • Licensure to practice as a physical therapist in California
  • At least two years of experience in home health or hospice care, preferably with a focus on physical therapy
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
  • A commitment to delivering patient-centered care that respects the unique needs and preferences of each individual
