Key Responsibilities- Provide Direct Patient Care: Assess, plan, implement, and evaluate patient care to ensure optimal health outcomes.
- Collaborate with Healthcare Team: Work closely with physicians, nurses, and other healthcare professionals to coordinate patient care and ensure seamless transitions.
- Develop and Implement Care Plans: Create and implement individualized care plans that address the unique needs and goals of each patient.
- Monitor and Document Patient Progress: Continuously monitor patient progress, document observations, and report any changes or concerns to the healthcare team.
- Provide Education and Support: Educate patients and their families on health conditions, treatment options, and self-care techniques to promote optimal health outcomes.
- Participate in Quality Improvement Initiatives: Contribute to quality improvement initiatives and participate in ongoing education and training to stay up-to-date on best practices and industry developments.
- Education: Graduate of an accredited registered nursing program.
- Experience: At least six months of experience as a Registered Nurse in an ambulatory care setting.
- Certifications: Current license to practice as a Registered Nurse in California and current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification.
- Skills: Excellent communication and interpersonal skills, ability to work effectively in a fast-paced environment, and strong attention to detail.
