Job Summary:

Madison Health & Rehabilitation is seeking a skilled Wound Care Nurse - RN to join our team on weekends. As a Wound Care Nurse, you will play a critical role in managing the skin program for our facility, ensuring the highest level of care for our residents.

Key Responsibilities:

  • Conduct weekly skin assessments to identify risks and develop appropriate interventions and documentation.
  • Measure and document all skin areas weekly, and initiate and update care plan information regarding skin.
  • Communicate with physicians, residents, family members, charge nurses, and other interdisciplinary team members regarding resident status as appropriate.
  • Chair the skin management meeting on a weekly basis and provide necessary information to the interdisciplinary team to develop interventions to prevent avoidable skin breakdown.
  • Monitor and adjust equipment use per facility protocol and resident need, and maintain ongoing records of equipment use.
  • Participate in quality improvement activities as assigned, and perform additional duties as required.

Requirements:

  • RN with a minimum of 3 years related experience, or equivalent combination of education and experience.
  • Current licensure by the state, and good standing.
  • Ability to relate positively and favorably to residents and families, and to work cooperatively with other associates.
  • Organizational and efficiency skills, with the ability to educate staff as needed.
