Job Summary

We are seeking a skilled Pediatric Social Worker to join our team at vTech Solution. As a key member of our care management team, you will provide psychosocial support to patients and families in a consult-based service.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify discharge plans and provide resource navigation and coordination of care.
  • Develop and implement brief clinical interventions to address patient needs and engage in safety planning as necessary.
  • Collaborate with RN Case Managers and the care team to ensure safe and ethical discharge plans.
  • Work closely with the Children's Hospital team to support pediatrics, PICU, NICU, and maternity units.
Requirements
  • MSW degree and 2 years of experience in acute care and pediatrics.
  • Alaska Social Work license not required, but experience with Epic is a plus.
  • Ability to work 5x8 days (40 hours) with a guaranteed 72 hours per 2 weeks.
