Family Treatment Court Case Manager

ROLE SUMMARY
This role is dedicated to overseeing case management for participants involved in the Family Treatment Court (FTC) program.
KEY RESPONSIBILITIES
• Ensure participants are well-informed about program expectations and service schedules, providing them with necessary materials and contact details.
• Create tailored case plans based on participant evaluations, incorporating FTC requirements, available community resources, and individual aspirations; these plans will align with existing DFCS case plans and will be updated as new evaluations occur.
• Maintain consistent communication with participants, meeting face-to-face (either in-person or virtually) at least once a week, supplemented by phone, text, or email interactions.
• Regular check-ins with participants will cover:
o Compliance with treatment
o Results from drug screenings
o Reports from surveillance
o Participation in NA/AA groups, support groups, and contact with sponsors
o Incentives and sanctions
o Updates on employment and education
o Housing issues
o Child visitation matters
o Additional resources/services as needed
• Facilitate referrals to community resources as necessary; assist participants with applications, enrollments, and scheduling appointments.
• Stay informed about community support options, including job and educational opportunities, housing, inpatient treatment, support groups, childcare, and DHS OFI services such as TANF, CAPS, and Medicaid.
• Provide crisis intervention for participants experiencing lapses or relapses, and assist with referrals for therapeutic responses, including inpatient treatment when required.
• Organize treatment groups and maintain communication with service providers regarding participant attendance and engagement.
• Coordinate random drug testing with the Accountability Court Lab, ensuring ongoing communication regarding participant compliance and test outcomes.
• Conduct meetings for Phase Up/Graduation with participants to review necessary documentation and requirements for the Phase Up/Graduation process.
• Procure incentives and assemble gift packages for Phase Up and Graduation events.
• Under the guidance of the FTC Coordinator, engage in team meetings and prepare updates for court staffing; lead court staffing in the absence of the FTC Coordinator.
• Assist the FTC Coordinator in compiling grant reports and court documentation.
• Utilize Case Worx or current case management software for documentation; ensure all records are current and maintain both electronic and physical participant files.
• Update GPRA evaluations in SPARS biannually as required by grant stipulations.
• Act as a liaison for communication during hearings, facilitating updates among team members.
• Attend and contribute to conferences, court hearings, meetings, and committees as necessary; participate in other agency meetings such as DFCS Family Team meetings and school meetings to support participants.
REQUIRED QUALIFICATIONS
• Associate degree in Social Science, Criminal Justice, or a related discipline; Bachelor's degree is preferred.
• A minimum of three (3) years of experience in social services or criminal justice, or an equivalent combination of education and experience.
• Valid driver's license from the State of Georgia.
