Key Responsibilities:
Patient Assessment:
- Conduct comprehensive admission assessments to establish a clear understanding of the patient's condition.
- Document thorough information that integrates the care plan, interventions, and outcomes, reflecting the patient's physical, mental, and emotional health.
- Perform regular reassessments on a shift-to-shift basis.
- Complete transfer and discharge documentation, evaluating the patient's needs to determine the appropriate level of care.
Medication Administration:
- Adhere to the five rights of medication administration, ensuring clear communication with patients and their families regarding the medications being administered.
- Assess patients prior to medication administration, taking into account allergies, vital parameters, potential side effects, and effectiveness, intervening as necessary.
- Manage pain based on patient feedback and assessment findings, documenting responses in accordance with hospital standards.
- Identify high-risk medications and utilize references as needed.
Patient Safety:
- Evaluate patients for potential safety risks upon admission.
- Follow hospital policies and procedures to ensure patient safety.
- Initiate interventions based on identified risk factors.
Patient and Family Education:
- Assess the educational needs of patients and their families.
- Develop and deliver tailored instruction using appropriate teaching methods.
- Evaluate and document the responses of patients and families to educational efforts.
Delegation and Leadership:
- Apply the five rights of delegation in coordinating and supervising nursing care delivery.
- Maintain accountability for delegated tasks and clarify the roles of team members.
- Foster a positive work environment through teamwork and personal example.
- Address unit staffing needs proactively in relation to patient acuity and census.
Skin Care Management:
- Assess the condition of patients' skin upon admission and throughout their stay.
- Implement preventive measures for patients at risk of skin breakdown.
- Stage pressure ulcers according to established guidelines.
Care Delivery Standards:
- Adhere to nursing standards of practice, including assessment, care planning, interventions, and evaluation.
- Conduct walking rounds for shift reports and introductory rounds with assigned patients.
Qualifications:
Education:
- Associate's degree in nursing required.
Licensure:
- Valid nursing license to practice in the State of Connecticut.
Experience:
- Experience ranging from zero to six months is acceptable.
Certifications:
- Basic Cardiac Life Support (CPR) certification is required within three months of hire.
Working Conditions:
Physical Requirements:
- Significant manual skills and coordination are necessary.
- May require lifting up to 50 lbs.
- Exposure to various environmental factors including noise and odors.
