About the Role

As a Case Manager, you will be part of an interdisciplinary team dedicated to delivering exceptional patient care and outcomes.

Key Responsibilities

  • Collaborate with healthcare professionals to develop and implement individualized treatment plans, ensuring seamless care transitions and optimal patient outcomes.
  • Conduct thorough assessments, identify patient needs, and coordinate care to achieve goals and objectives.
  • Communicate effectively with patients, families, and caregivers to ensure informed decision-making and timely discharge planning.
  • Participate in weekly patient care team conferences to review progress, address challenges, and refine treatment plans.
  • Review patient records, identify potential comorbid conditions, and communicate findings to the healthcare team.

Requirements

  • Current licensure as a Registered Nurse, Social Worker, or Healthcare Professional (Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist).
  • Certification in Case Management or Rehabilitation Nursing is preferred.
  • Minimum 2 years of experience in an inpatient setting, preferably in acute or rehabilitation hospitals.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
