Rehabilitation Case Manager

As a Case Manager, you will be part of an interdisciplinary team dedicated to delivering exceptional patient care and outcomes.
Key Responsibilities
- Collaborate with healthcare professionals to develop and implement individualized treatment plans, ensuring seamless care transitions and optimal patient outcomes.
- Conduct thorough assessments, identify patient needs, and coordinate care to address complex medical and social issues.
- Communicate effectively with patients, families, and caregivers to ensure informed decision-making and timely discharge planning.
- Participate in weekly patient care team conferences to review progress, address challenges, and refine treatment plans.
- Review patient records, identify potential comorbid conditions, and communicate findings to the healthcare team.
Requirements
- Current licensure as a Registered Nurse, Social Worker, or Healthcare Professional (Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist).
- Certification in Case Management or Rehabilitation Nursing is preferred.
- Minimum of 2 years of experience in an inpatient setting, preferably in acute or rehabilitation hospitals.
- Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
