Job SummaryWe are looking for an individual who is passionate about delivering high-quality patient care and has a commitment to improving the health and well-being of others. The ideal candidate will possess strong clinical skills, excellent communication and interpersonal abilities, and a proven track record of success in a Family Med, Community Health, or Outpatient setting.
The successful applicant will have the opportunity to work alongside a talented team of healthcare professionals, conducting comprehensive health assessments, developing treatment plans, and prescribing medications as needed. In addition to providing exceptional patient care, the role will also involve maintaining accurate and up-to-date medical records, participating in quality improvement initiatives, and collaborating with other healthcare staff members to ensure the highest level of care.
Responsibilities- Conduct comprehensive health assessments, including physical exams, medical histories, and diagnostic tests.
- Develop treatment plans and prescribe medications as needed.
- Provide patient education and counseling on disease prevention, healthy lifestyle choices, and other health-related topics.
- Collaborate with other healthcare professionals to ensure the highest level of patient care.
- Maintain accurate and up-to-date medical records and documentation.
- Participate in quality improvement initiatives and other clinical projects as needed.
- A license to practice as a Nurse Practitioner or Physician Assistant.
- Previous experience in a Family Med, Community Health, or Outpatient setting.
- Strong clinical skills and knowledge of primary care practices and procedures.
- Excellent communication and interpersonal skills.
- Able to work well in a team environment and collaborate effectively with other healthcare professionals.
- Ability to maintain accurate and up-to-date medical records.
