Hospice Admission Nurse RN

Job Summary:
We are seeking a compassionate and skilled Registered Nurse RN to join our Hospice Care team as an Admissions Nurse. As a key member of our interdisciplinary team, you will be responsible for assessing patients and their families, providing education and support, and facilitating the admission process to our hospice care services.
Responsibilities:
- Travel to patient sites to assess patients and their families, providing education and support as needed.
- Partner with our Account Representative staff to identify new referral opportunities and build relationships with healthcare providers.
- Assess patients' physical and emotional conditions, determining their readiness for hospice care and identifying any special needs or requirements.
- Communicate with patients' primary care physicians and other healthcare providers to ensure seamless care transitions.
- Obtain orders and initiate documentation for crisis care as needed.
- Evaluate patients' eligibility for hospice care in a timely manner, obtaining approval from our physician and the patient's primary care physician as required.
Requirements:
- Associate's degree in Nursing.
- Minimum 3 years of Registered Nurse experience in home health and/or acute care nursing, including 1 year of experience in a hospital setting.
- Active Florida Registered Nurse license and Basic Life Support Certification.
- Valid Florida Driver's License and reliable vehicle.
- Personal automobile insurance.
Preferred Qualifications:
- Bachelor of Science in Nursing.
- Certified Hospice and Palliative Nursing certification.

Schedule:
Full-time positions available with varying schedules, including every weekend plus 2 variable weekdays, 11:30am-10:00pm Monday-Friday, and 10am-6:30pm Monday-Friday with 2 weekend days per month commitment.
