Position Overview:
The Center Medical Director at Oak Street Health is responsible for guiding the clinical team in delivering exceptional patient care while fostering a supportive work environment. The primary objective is to ensure patient satisfaction, health, and prevention of hospital admissions.
This role involves practicing primary care within our clinics, dedicating 80% of time to clinical duties and 20% to administrative responsibilities.
The CMD will undertake vital leadership tasks, which encompass exemplifying exemplary primary care practices, mentoring and advising clinic providers, and engaging in significant organizational strategic initiatives.
Key Responsibilities:
The Center Medical Director serves as the medical authority for their respective center.
In this capacity, the CMD will be tasked with the following:

Collaborate closely with the Practice Manager and Outreach Manager/Director to fulfill organizational goals:

Facilitate the execution of strategic initiatives across Oak Street Health.
Implement directives from regional leadership in conjunction with other CMDs to achieve regional objectives.
Disseminate organizational updates to care team members while elevating care team feedback and suggestions to the broader organization.
Provider OversightAssist in the onboarding process for new providers at the center.
Demonstrate exemplary medical care, adherence to care models, and achievement of performance metrics.
Ensure effective implementation of the Oak Street Health care model by preparing care teams for daily huddles, patient care planning, admission prevention efforts, and clinical documentation.
Guarantee that the medical services provided by each provider and care team are evidence-based, promoting patient health and minimizing hospitalizations.
Familiarize, instruct, and apply Oak Street's clinical guidelines at both the individual provider and center levels.Support care teams in delivering high-quality preventative care.
Provide continuous education and assistance to providers in the value-based care model.
Identify and address underlying issues when center performance metrics are not met.
Leadership and Mentorship
Foster a culture of growth and development among providers and care teams.
Actively listen to feedback from team members and address concerns through constructive solutions.
Conduct performance evaluations and collaborate on provider goal-setting and achievement plans.
Workplace Culture
Recognize and celebrate team member achievements while identifying areas for improvement.
Implement best practices for team collaboration.
Promote a positive and joyful workplace culture.

Qualifications:
Ideal candidates will possess:
M.D., D.O., or A.P.N or P.A. qualifications.
Board certification in Internal Medicine or Family Medicine required for M.D. or D.O.; Board certification in Family, Adult, or Gerontology for A.P.N.
Experience in clinical leadership roles, mentoring and guiding providers to excel in patient care.
Proficiency in utilizing metrics to drive medical care quality and improvement projects.
Passion for healthcare and a desire to contribute to a rapidly growing organization.Awareness of leadership capabilities and a willingness to share insights with a dynamic, innovative healthcare team.
Commitment to serving older adults and low-income populations.Ability to navigate uncertainty and solve problems effectively.
Capacity to maintain composure during challenging situations, guiding others toward effective solutions. A minimum of 5 years of experience in outpatient practice is preferred.
Fellowship training in Geriatrics, additional professional degrees (e.g., M.B.A., J.D., M.P.H.), and prior executive experience are welcomed but not mandatory.Authorization to work in the U.S.
