Responsibilities- Provide patient-centered clinical services, including medication information, education, and counseling.
- Develop and implement patient care plans in collaboration with healthcare teams.
- Conduct thorough medication reviews and provide recommendations for optimal medication therapy.
- Collaborate with healthcare providers to ensure seamless care transitions and optimal patient outcomes.
- Stay up-to-date with the latest clinical guidelines, regulations, and best practices in pharmacy care.
- Doctorate in Pharmacy (PharmD) from an accredited institution.
- Current licensure as a pharmacist in the state of practice.
- 1 year of recent experience in a hospital or clinical pharmacy setting.
- Strong knowledge of pharmaceutical calculations, laws, and regulations.
- Excellent communication and interpersonal skills.
- A competitive hourly rate of $90.97 to $100.06.
