  • Independent Activities: Assist clients with light housekeeping, shopping, laundry, and meal preparation, as per the Plan of Care.
  • Transportation: Accompany clients to appointments, unless otherwise specified in the Plan of Care.
  • Medication Reminders: Provide assistance with self-administered, pre-drawn, and medication reminders, as per the Plan of Care.
  • Infection Control and Safety: Understand and maintain infection control and safety measures.
  • Reporting Changes: Report any changes in the client's condition to the Care Manager or BOM.
  • Documentation: Complete Activity Records and accurately document the care provided.
  • Team Collaboration: Perform all activities as directed by the Care Manager or BOM and maintain a positive relationship with clients and families.

Requirements:

  • Qualifications: Must have at least one year of certifiable experience in home care, private-duty home care setting, community health agency, hospital, or med-surg clinical rotation in a nursing school, or have at least one year of verifiable work experience in an unrelated field and, if in an employed state, have successfully completed a Home Health Aide Training course approved by the Vice President of Home.
  • Communication: Demonstrate an ability to effectively communicate in English, and/or the predominant language of the population being served, and must be able to read and write.
  • Competency Evaluation: Pass the Spring Hills Home Care Services Competency Evaluation.
  • Criminal Background Check: Pass a criminal background check.
  • Medical Requirements: Meet all medical requirements.
  • State Requirements: Meet all state requirements.
  • Transportation: Have reliable transportation and a method of communication.
  • Dependability: Be punctual, dependable, and demonstrate sound judgment.
