Job Summary

SpecialCare Hospital Management is seeking a highly skilled and compassionate Detox Intake Coordinator to join our team. As a key member of our intake team, you will be responsible for screening and assessing patients to ensure they meet our service criteria, as well as developing personalized discharge plans for those who have completed treatment.

Responsibilities:

  • Conduct thorough screenings and assessments of patients via phone and in-person inquiries, initiating insurance verification and scheduling assessments and intakes.
  • Complete all necessary paperwork clearly and concisely, adhering to policy and procedure.
  • Effectively communicate with hospital staff, including physicians and nursing staff, regarding patient assessment, continued stay, and discharge planning.
  • Maintain HIPAA confidentiality and adhere to all relevant policies and procedures.
  • Round on patients daily, following policy and procedure, and conduct follow-up calls as needed.
  • Develop and implement discharge and aftercare plans in collaboration with patients and their families.
  • Report daily census to the corporate office and perform community outreach telephone calls to various agencies.
  • Complete bio-psychosocial assessments and other related duties as determined by supervisor.

Qualifications:

  • Bachelor's degree in Human Services or a related field, with at least 2 years of experience in social service and/or clinical service work.
  • Knowledge of addiction and/or dependence, with a strong understanding of substance abuse and withdrawal.
  • Substance abuse counselor and/or case manager experience, with a background in MAT treatment or detox, is highly desirable.
  • Ability to work effectively as a team player, with minimal supervision, and to assist and support others as needed.
  • Strong problem-solving skills and ability to handle stressful situations.
  • Past experience in a hospital setting and hospital management is preferred.
  • Satisfactory drug screen and criminal background check.
