We are seeking a skilled Nurse Practitioner to join our team at St. Luke's Health System. As a key member of our Home Health team, you will provide high-quality patient care and support our mission to deliver exceptional healthcare services.
Key Responsibilities- Provide comprehensive care to patients with a new diagnosis, medications, or have been discharged from ER, require respiratory treatment or wound care.
- Ensure patients are progressing with their care plan, provide education related to their disease process, and assess physical and psychological needs to improve quality of life.
- Coordinate resources and educate patient and patient's family when it comes time to discharge.
- Education: ASN or BSN Degree
- Experience: 1 year of RN experience; Hospice or Acute Care experience helpful
- Licenses/Certifications: Current RN License in the State of Practice; Current Basic Life Support (BLS) Certification
- Valid Driver's License and proof of Auto Insurance
