Job Title: Speech Therapist

We are seeking a compassionate and dedicated Speech Therapist to join our Home Health team in St. Lucie County, Florida.

Job Summary:

This is a Per-Diem position for an individual who is passionate about providing quality home health care to patients in need.

Responsibilities:
  • Perform initial and ongoing assessments to evaluate patients' health status and develop individualized care plans.
  • Administer occupational therapy treatment in accordance with physician orders and plan of care.
  • Collaborate with other members of the healthcare team to provide coordinated and holistic care to patients.
  • Ensure accurate and timely documentation of patient care and progress.
  • Coordinate and communicate with healthcare providers, families, and caregivers to ensure continuity of care.
Qualifications:
  • Valid and current ST license in the state of Florida.
  • Minimum of 1 year of home health or related experience, preferred.
  • Strong clinical and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and prioritize tasks efficiently.
