Objective:
The Rehabilitation Specialist conducts comprehensive evaluations of patient health records, performs initial diagnostics, formulates rehabilitation strategies, administers therapeutic interventions, tracks patient progress, and meticulously documents all treatment activities (including discharge planning and patient education) to enhance overall functionality, alleviate discomfort, improve or sustain flexibility and strength, and accomplish various medical and therapeutic objectives.
Key Responsibilities:
- Communicates the patient's condition to the referring physician and other members of the healthcare team.
- May act as a clinical mentor for physical therapy students, residents, and fellows specializing in specific areas of physical therapy.
- Maintains professional licensure through ongoing education and professional development to meet the requirements for license renewal.
- Conducts physical therapy evaluations, reassessments, and treatments while supervising physical therapist assistants in the care of pediatric, adolescent, adult, and geriatric patients referred by physicians. Develops a comprehensive care plan including objectives and treatment strategies based on physician referrals, the nature of the injury or illness, and assessment results. Adjusts the care plan as necessary based on the patient's response to treatment and any changes in their condition. Collaborates with PTAs to modify patient programs as needed.
- Rotates through designated treatment areas as required to meet organizational needs.
- Participates actively in staff meetings and in-service training sessions.
- Maintains thorough documentation for all rehabilitation services provided, including initial assessments, follow-up visits, and discharge summaries in accordance with departmental standards. Co-signs documentation from physical therapist assistants biweekly.
- Completes necessary billing and statistical records to accurately reflect the services provided to patients.
- Provides verbal and/or written home care instructions to patients and/or their families when appropriate.
- Schedules patients to ensure productivity levels meet or exceed the standards established by the organization while ensuring high-quality patient care.
Educational/Knowledge Requirements:
- Graduate of an accredited program recognized by the American Physical Therapy Association (APTA).
Licensure, Certifications, and Clearances:
Active license in the practicing state is required. CPR certification is required based on AHA standards, which include both theoretical and practical components within 30 days of hire.
- Basic Life Support (BLS)
- Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
- Physical Therapist (PT)
- Act 34
