Home Care Licensed Practical Nurse Job Summary

We are seeking a skilled and compassionate Home Care Licensed Practical Nurse to join our team in Green Bay. As a Home Care LPN, you will provide high-quality nursing care to patients in their homes, working under the supervision of a Registered Nurse.

Key Responsibilities:
  • Provide direct nursing care to patients in their homes, following the plan of care and in coordination with other healthcare team members.
  • Document patient visits thoroughly and accurately in the electronic medical record.
  • Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
  • Ensure confidentiality of patient health information.
Requirements:
  • Current LPN licensure in the state of practice.
  • Minimum of one year of acute care experience, with previous home care experience preferred.
  • Current CPR certification.
