Home Care Licensed Practical Nurse Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our home care team in Green Bay. As a Home Care LPN, you will provide high-quality clinical services to patients in their homes, working under the direct supervision of a Registered Nurse.

Key Responsibilities:

  • Provide direct nursing care to patients in their homes, following the plan of care and in coordination with other members of the healthcare team.
  • Document patient visits thoroughly and completely in the electronic medical record, utilizing the agency's electronic visit verification program.
  • Actively participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, and other clinicians.
  • Ensure confidentiality of patient health information and maintain a professional demeanor at all times.

Requirements:

  • Current LPN licensure in the state of practice.
  • A minimum of one year of acute care experience, with previous home care experience preferred.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
