Job Summary:
We are looking for a compassionate and experienced RN to provide high-quality care to patients in their homes. As a Home Health Admission RN, you will be responsible for conducting clinical assessments, developing care plans, and collaborating with interdisciplinary teams to ensure seamless patient care.
Responsibilities:
- Conduct thorough clinical assessments of patients at time of admission
- Develop and implement individualized care plans
- Collaborate with healthcare teams to ensure continuity of care
- Precept and mentor new staff members
Requirements:
- BSN required; ADN candidates must acquire BSN within 3 years
- 2.5 years of recent skilled adult home health experience required
- OASIS documentation experience and proficiency required
