Job Summary

We are seeking a dedicated Rehabilitation and Support Specialist to join our team at Winds of Change. As a key member of our staff, you will play a vital role in providing crucial support for individuals on their well-being journey.

Key Responsibilities
  • Assist clients in achieving their recovery goals through personalized support and guidance.
  • Collaborate with Case Managers and Therapists to develop and implement effective recovery plans.
  • Provide transportation for clients to their appointments and facilitate recovery-based group activities.
  • Assess and evaluate barriers that impact independent or community living, and develop strategies to overcome them.
  • Improve communication skills and identify strategies for managing symptoms.
  • Assist in developing social support systems and adapt skills for various areas of life.
  • Offer immediate crisis intervention when necessary and maintain detailed documentation on a daily basis.
  • Meet productivity standards and adhere to HIPAA regulations and maintain confidentiality.
  • Cover shifts at group homes as needed, including day, evening, and overnight shifts.
  • Schedule and attend necessary meetings and demonstrate regular attendance.
Requirements
  • Familiarity with HIPAA regulations and a strong commitment to maintaining confidentiality.
  • Proactivity and the ability to anticipate the needs of clients.
  • Sound judgment and accuracy in decision-making.
  • A positive attitude and passion for the work.
