Responsibilities:

  • Provide primary care services in post-acute care facilities, including skilled nursing and assisted living
  • Drive high-quality, safe, and cost-effective patient care
  • Collaborate with interdisciplinary teams and follow CMS guidelines
  • Work closely with hospice, palliative care, and home care agencies for safe patient transitions
  • Provide oversight, guidance, and mentorship to clinicians within facilities

Compensation and Benefits:

  • Productivity-based model (fee for service) with uncapped earning potential
  • Estimated annual salary range of $206,000 to $277,000
  • No primary night call with support from our in-house call group
  • Paid professional liability insurance including tail coverage
  • Industry-leading onboarding program with EMR, MIPS, billing and coding training, and support from experienced clinical educators

Position Details:

  • Sign-on bonus: $10,000
  • Schedule: Monday through Friday
  • Patient volume: 20 patients per day

Qualifications:

  • Background in providing primary care in internal medicine, family medicine, emergency medicine, urgent care, PM&R, or geriatrics
  • Ability to work flexible schedules
