We are seeking a highly skilled Clinical Pharmacist to join our Inpatient Pharmacy team on a per diem basis. The ideal candidate will have a strong background in medication management and a passion for delivering exceptional patient care.
Key Responsibilities- Provide medication management services in a collaborative manner, including reviewing patient medication history, educating patients on drug therapy, and developing therapeutic plans.
- Maintain and update patient profiles with demographics, diagnosis, allergies, and current medications.
- Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interaction.
- Provide drug information to healthcare professionals and patients.
- Document clinical interventions and counsel patients on discharge medications as appropriate.
- Doctor of Pharmacy (Pharm.D) degree or Bachelor of Science in Pharmacy (BSPharm) degree with 3 years hospital experience.
- Licensure with the Massachusetts Board of Registration in Pharmacy.
- ASHP Accredited Residency in Pharmacy Practice (PGY1) is recommended.
