Key Responsibilities
  • Provide evidenced-based care to patients, including accurate assessment, diagnosis, treatment, and management of health problems.
  • Work collaboratively with the clinical care team to ensure the delivery of high-quality, patient-centered care.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in clinical care team huddles and contribute to the development of care plans.
  • Stay up-to-date with the latest clinical guidelines and best practices in primary care.
Requirements
  • Master's degree in Family Nurse Practitioner program or Master of Science in Physician Assistant Studies.
  • Current National Board Certification and State of Employment license to practice in the Advanced Practice role.
  • Minimum of 1 year of recent Primary Care experience as a Nurse Practitioner or Physician Assistant.
  • Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
  • Effective verbal, written, and electronic communication skills.
  • Outstanding organizational skills and ability to multi-task.
