Job Summary:

We are looking for a dedicated and compassionate Per Diem Nurse to provide high-quality patient care in a home health setting. The ideal candidate will have excellent communication skills, a strong work ethic, and the ability to work independently with minimal supervision.

Responsibilities:
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Conduct thorough assessments, identify patient needs, and develop effective care plans.
  • Provide education and support to patients and their families to promote healthy outcomes.
Requirements:
  • Current RN licensure in the State of Ohio.
  • Minimum 2 years of experience in home health or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently with minimal supervision.
  • Strong problem-solving and critical thinking skills.
