Job Summary:

The Clinical Pharmacist will provide comprehensive clinical pharmacy services for patients receiving home infusion therapy. This role requires a strong understanding of medication therapy and the ability to work collaboratively with healthcare teams to ensure patient safety and optimal outcomes.

Responsibilities:
  • Reviews prescriptions to ensure accuracy and determines formulas and ingredients needed for dispensing.
  • Collaborates with healthcare teams to assess patient suitability for home infusion therapy and develops individualized plans of care.
  • Monitors patient progress and communicates with prescribing physicians to ensure timely and effective treatment.
  • Acts as a clinical resource for healthcare teams on medication use, potential interactions, and side effects.
  • Provides education and consultation to patients and caregivers on medication use and management.
  • Prepares and dispenses medications according to NELC policies and procedures.
  • Monitors physician orders for drug interactions, duplication of therapy, and appropriateness of drug orders.
  • Detects and evaluates adverse drug reactions and complications associated with home infusion therapy.
  • Documents patient progress and communicates with healthcare teams to ensure seamless care coordination.
Requirements:
  • Bachelor of Science degree from an accredited school of pharmacy.
  • Registered Pharmacist, in good standing, with a valid license.
  • Excellent interpersonal and organizational skills.
  • Ability to apply mathematical operations to tasks such as dosage calculation and data analysis.
  • Ability to communicate effectively with patients, caregivers, and healthcare teams.
Preferred Qualifications:
  • Doctorate of Pharmacy degree.
  • Knowledge of HIPAA Privacy and Security Regulations and JCAHO accreditation process.
  • Working knowledge of microcomputer systems, patient documentation systems, and automated pharmacy systems.
  • Two years' experience in intravenous admixing and stability and compounding.
