Job Title: RN Home Health Baylor

About the Role:
  • Provide skilled nursing visits to patients in their homes, assessing their needs and developing personalized care plans.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
  • Conduct ongoing assessments and evaluations to adjust care plans as needed, ensuring patients receive the best possible care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient progress and care plans accurately and thoroughly, maintaining high standards of medical record-keeping.
Requirements:
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum one year of nursing experience, preferably in med-surg, ICU, ER, or acute care settings.
  • Current and unrestricted RN licensure.
  • CPR certification.
  • Strong organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.
