About the Role

La Clinica de La Raza is seeking a skilled Behavioral Health Case Manager I to join our team. As a key member of our primary care team, you will work closely with patients, families, and healthcare providers to address the social determinants of health that impact our patient population.

Key responsibilities include:

  • Developing strong relationships with patients and families to support their health and well-being
  • Providing patient education, motivational interviewing, and emotional support
  • Advocating for patients and connecting them to community resources and services
  • Collaborating with healthcare providers and other stakeholders to ensure timely and coordinated care
  • Using Electronic Health Record (EHR) and population management tools to collect data and communicate with healthcare providers

Requirements include:

  • Demonstrated cultural competence and proactive self-reflection on positionality and incorporation of antiracism approaches to clinical care
  • Knowledgeable in the principles, practices, and techniques of social work and basic psychological theory and practices
  • Familiarity with local resources and regulations, including behavioral health care settings
  • Familiarity with Motivational Interviewing and ability to quickly identify risk factors as they relate to patient safety
  • Ability to work across cultures and demonstrate support of diversity, equity, and inclusion
