Community Care Supervisor

Department: Behavioral Health Services - Adults
Status: Three (3) 3/4-Time, Non-Exempt
Hours: 30-39 hours per week
Under general supervision, the Community Care Supervisor plays a crucial role in delivering rehabilitative and restorative treatment services. This includes crisis intervention, therapeutic support, assessment evaluations, and comprehensive case management. Responsibilities involve engaging with clients in various settings, providing referrals, and offering counseling on diverse issues while delivering one-on-one behavioral support.
Key Responsibilities:- Facilitates activities that align with the objectives of the rehabilitative group and individual client plans.
- Adheres to State and County Mental Health guidelines for all services rendered.
- Engages in meaningful conversations with clients to reinforce positive behaviors and enhance social interactions.
- Serves meals and shares dining experiences with clients to model appropriate behaviors as per rehabilitative or behavioral plans.
- Plans and executes regular activities or outings aimed at skill development.
- Provides medication support to clients as necessary and ensures all required documentation is completed.
- Care Manager I: Less than two (2) years of full-time experience in a mental health-related field providing direct services.
- Care Manager II: At least two (2) years but less than four (4) years of full-time experience in a mental health-related field providing direct services.
- Care Manager III: A minimum of four (4) years of full-time experience in a mental health-related field providing direct services, with up to two (2) years of experience substitutable by relevant education.
- Flexibility to work across various agency programs as needed.
- Occasional adjustment of work schedule to accommodate individual client needs.
- Possession of a valid driver's license and current auto insurance.
- Successful completion of a background check.
- Certification in CPR and First Aid.
- Completion of a TB Exam and Physical upon employment.
Comprehensive onboarding that includes training in CPR/First Aid, Crisis Communication, Trauma-Informed Care, Motivational Interviewing, and Collaborative Problem Solving.
