Job Summary

Montefiore Medical Center is seeking a skilled Home Health Nurse to join our team. As a Home Health Nurse, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Act as a liaison between patients, families, and physicians to ensure seamless care coordination.
  • Provide regular updates on patient condition to families and physicians.
  • Ensure all in-home medical equipment is in working order to prevent any potential risks.
  • Document all details of patient's condition, progression, and regression activity to track their progress.
  • Oversee required physical therapy or rehabilitation exercises to help patients regain their strength and mobility.
  • Contact emergency services when patients require immediate hospitalization.
  • Possess knowledge of Home Care reimbursement processes to ensure accurate billing and payment.
  • Possess knowledge of OASIS (Outcome and Assessment Information Set) to track patient outcomes and quality of care.
  • Have reliable transportation to travel to and from patient homes.
  • Be able to walk up and down six flights of stairs as needed to access patient homes.
Requirements
  • Minimum of one year of hospital Med Surg experience.
  • One year of Home Health experience, with Certified Home Care (CHHA) experience preferred.
  • NYS RN License and Registration in good standing.
  • Bachelor's degree.
  • Infection Control certificate.
  • Valid Driver License, Registration, and Automobile Insurance.
  • Knowledge of a second language is a plus.
