Job Title: Physical Therapist - Home Health

VNA Health Group is seeking a skilled Physical Therapist to join our team in Englewood, New Jersey.

Job Summary:

We are looking for a compassionate and dedicated Physical Therapist to provide high-quality care to our patients in their homes or alternate community settings.

Responsibilities:
  • Conduct thorough assessments and evaluations of patients to identify their physical therapy needs.
  • Develop and implement individualized plans of care to help patients achieve their goals and improve their overall well-being.
  • Provide skilled physical therapy interventions, including exercises, modalities, and education, to help patients manage their conditions and prevent further decline.
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care and coordination.
  • Document all patient interactions, progress, and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices and evidence-based care.
Requirements:
  • Current Physical Therapy licensure in New Jersey.
  • 2-3 years of prior clinical experience, preferably in home health or community-based settings.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Valid New Jersey driver's license.
