Job Summary

Albany Medical Center is seeking a skilled Nurse Case Manager to join our team in the Inpatient Rehabilitation department. As a key member of our interdisciplinary team, you will play a vital role in coordinating patient care across the continuum of care, ensuring timely and appropriate interventions to achieve optimal patient outcomes.

Key Responsibilities
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans, ensuring seamless transitions across the care continuum.
  • Proactively monitor patients' clinical progress, utilizing evidence-based guidelines and patient care standards to identify opportunities for improvement.
  • Provide education and support to patients and families, promoting continuity of care and optimal patient outcomes.
  • Facilitate communication among healthcare team members, ensuring effective care coordination and minimizing delays in care.
  • Participate in quality improvement initiatives, contributing to the development and implementation of strategies to enhance patient care and outcomes.
Requirements
  • Bachelor's degree in Nursing or related field.
  • 1-3 years of clinical experience in an acute care setting, preferably in case management or utilization management.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to adapt to changing priorities and deadlines, with a focus on delivering high-quality patient care.
  • Basic knowledge of computer systems and software applications, with experience in electronic medical records and other healthcare technologies.
