As a Hospice Care Professional with LifeBridge Health, you will play a vital role in ensuring that all services are delivered in accordance with the organization's Mission Statement and S.P.I.R.I.T. Values. You will work as a para-professional, trained to provide personal care and other healthcare needs of patients under the direction of a Registered Nurse.
Responsibilities- Deliver high-quality patient care and support services in accordance with established standards and protocols.
- Work collaboratively with the healthcare team to ensure seamless care coordination and communication.
- Provide emotional support and companionship to patients and their families.
- Assist with daily living activities, such as bathing, dressing, and grooming.
- Monitor and report any changes in patient condition to the Registered Nurse.
- High School Diploma or equivalent required.
- Successful completion of a Certified Nursing Assistant (CNA) course required.
- Previous experience as a CNA/Home Health Aide, preferably in a Medicare Certified Program, preferred.
- CPR certification required.
- Ability to communicate effectively with patients and families, both verbally and in writing.
- Ability to adapt to changes in schedules, assignments, and geographical locations.
