Key Responsibilities

We are seeking a skilled and compassionate individual to join our team as a Hospice Social Worker-MSW PRN. The ideal candidate will have a strong background in social work, excellent communication skills, and the ability to work effectively in a fast-paced environment. Key responsibilities will include:

  • Conducting thorough psychosocial assessments of patients and families/caregivers in a timely manner
  • Counseling patients and caregivers on stress management and coping strategies
  • Providing accurate and timely documentation of patient services
  • Serving as a liaison between patients and families/caregivers and community agencies

Salary Range: $60,000 - $80,000 per year, depending on experience.
